Background and Aims: Advance directive in mental healthcare intends to empower individuals with mental illness by defining their treatment preferences or proxy while they have capacity. It has been incorporated in the mental health legislations of several countries to a varying extent. However, its implementation in a resource limited country comes with its own pitfalls. In this article we review the advance directive in mental healthcare in India and other countries. Methods: Major medical, health and psychological literature databases including PubMed, MEDLINE and other grey literature related to mental health legislations of various countries were searched for relevant data (up to January 2019). Results: Advance directive in mental healthcare differs in terms of its approach, legal status and situations in which they can be overturned etc. In low resource countries, issues with its implementation, monitoring and cost exist. Therefore, although advance directive in mental healthcare can safeguard patientu2019s autonomy, its use in low income countries have to be prudently analyzed.
